What is the FSP

The medical-German exam for the German Approbation

Conducted by the Landesärztekammer of each Bundesland. Tests whether a foreign-trained doctor can work safely in clinical German.

  • Conducting body: Landesärztekammer (State Medical Chamber) of the Bundesland where you apply.
  • Purpose: Language clearance for the Berufserlaubnis / Approbation — medical German at roughly C1.
  • Prerequisite: A general German B2 certificate (telc / Goethe / TestDaF) is normally required to apply.
  • Format: Oral, ~60 minutes, three parts.
  • Part 1 — Anamnese: ~20-min patient history with a simulated patient.
  • Part 2 — Übergabe: presenting / handing the case to a colleague (Arzt-Arzt).
  • Part 3 — Patientenbrief: a structured written medical report.
  • Pass mark & fee: set per chamber — confirm locally before booking.

What's possible online for FSP — Fachsprachprüfung Medizin

~90%prep fidelity

The real FSP is taken in-person at your state Ärztekammer with a standardised actor-patient and a doctor-examiner. We can prepare you for it — we cannot replace it.

What you get online

  • AI patient simulation (Anamnese) — Whisper-powered German voice, ~50 standardised cases (Brustschmerz, Atemnot, Bauchschmerz, neurological deficit, etc.) with real-exam timing
  • Arztbrief / Dokumentation editor — structured templates + AI rubric grading against the official FSP rubric
  • Doctor-doctor handover practice — recorded monologue with AI examiner asking dynamic follow-up questions
  • Full transcript + per-criterion scoring after every mock

What stays in-person only

  • The actor-patient's eye contact and body language during the in-person Anamnese
  • The Chief Physician's real-time interruption style and mood
  • The official Zertifikat — that's issued only by your state Ärztekammer after the in-person exam

Why this prep still helps you pass

FSP is gated by your German clinical fluency and the structure of your patient presentation — both can be drilled to mastery online. Most candidates who fail the real FSP fail on language fluency or structure, not on the in-person dynamics. Our AI examiner is more relentless than a real one and never gets tired.

Each Ärztekammer (Hessen, NRW, Bayern, etc.) sets its own exact format and may emphasise different specialties. We cover the common core; check your specific Bundesland's syllabus for state-specific differences.

Frequently asked

The most asked FSP questions

What is the Fachsprachprüfung (FSP)?+
The FSP is the medical-German language exam foreign-trained doctors must pass to obtain the German Approbation (full medical licence). It tests whether you can communicate in clinical German at roughly C1 level — taking a patient history, presenting a case to a colleague, and writing a medical report.
Who conducts the FSP and is it the same in every Bundesland?+
The FSP is conducted by the Landesärztekammer (State Medical Chamber) of each Bundesland. The three-part structure (Anamnese, Arzt-Arzt-Übergabe, Patientenbrief) is broadly the same, but pass marks, fees and exact timing vary by state — always confirm with the chamber where you will apply.
What are the three parts of the FSP?+
Part 1 — Arzt-Patient-Gespräch: a ~20-minute patient history (Anamnese) with a simulated patient. Part 2 — Arzt-Arzt-Gespräch: presenting and handing over the case to a colleague (Übergabe). Part 3 — Patientendokumentation: writing a structured medical report (Patientenbrief / Arztbrief) in German.
What German level do I need for the FSP?+
You need general German at B2 (certified, e.g. telc/Goethe/TestDaF) plus medical German at roughly C1. The FSP itself tests the medical-German competence; the B2 general-language certificate is usually a prerequisite for the application.
How does the FSP relate to the Kenntnisprüfung?+
The FSP tests language; the Kenntnisprüfung (KP) tests medical knowledge. Doctors whose qualification is judged not fully equivalent must pass both — typically FSP first (for the Berufserlaubnis / language clearance), then the KP for the full Approbation.
Can telc Deutsch B2·C1 Medizin replace the FSP?+
In several Bundesländer the telc Deutsch B2·C1 Medizin certificate is accepted in place of the chamber's FSP. Acceptance is state-specific, so confirm with your Landesärztekammer before choosing that route — many candidates prepare for both.
How do your FSP mocks help?+
Our mocks model all three parts: native-voice Anamnese simulations, structured Übergabe prompts, and Patientenbrief writing tasks with examiner-style feedback on register, terminology (Fachbegriff vs Laienbegriff) and structure.
