What is FRCS Section 1

The senior-trainee knowledge exam before the UK consultant tier

Conducted by the Joint Committee on Intercollegiate Examinations (JCIE) on behalf of the four UK + Ireland surgical Royal Colleges, FRCS Section 1 is the final written assessment in specialty surgical training.

Conducting body: JCIE — on behalf of the RCSEng, RCSEd, RCPSG and RCSI.
Eligibility: at least 4 years post-graduation in your surgical specialty; UK ST6+ or overseas equivalent.
Mode: Computer-Based Test (CBT) — Pearson VUE delivery worldwide.
Structure: Paper 1 (~110 items · ~2 hrs) + Paper 2 (~135 items · ~2.5 hrs), same day.
Item types: Single Best Answer (SBA) + Extended Matching Questions (EMQ).
Marking: +1 correct, 0 otherwise — no negative marking, attempt every item.
Pass standard: equated scaled score (Angoff) — sectional pass required on BOTH papers.

FRCS Section 1 vs MRCS Part A — what's the difference?+

MRCS Part A is the entry-level surgical gate, sat by core surgical trainees in their first 1-2 years and pitched at generic surgical sciences. FRCS Section 1 is the final specialty fellowship written exam, sat near the end of higher specialty training (ST6-ST7+). MRCS opens the door to surgical training; FRCS opens the door to UK consultant practice.

Which surgical specialty streams does FRCS Section 1 cover?+

Nine specialty-specific FRCS exams run under the JCIE umbrella: General Surgery, Trauma & Orthopaedics, Urology, Otolaryngology (ENT), Plastic Surgery, Paediatric Surgery, Cardiothoracic Surgery, Neurosurgery and Oral & Maxillofacial Surgery. You sit the diet that matches your specialty — content, blueprints and pass standards differ between streams.

How do Section 1 and Section 2 fit together?+

Section 1 is the written knowledge paper (SBA + EMQ, ~250 items, 5 hours CBT). On passing Section 1 you become eligible to enter Section 2 — a clinical and oral (viva) exam testing applied judgement, communication and surgical decision-making. You must pass BOTH sections to be awarded the FRCS diploma.

How many attempts do I get?+

JCIE permits up to 4 attempts at Section 1 within a 7-year window from your first sitting. If you don't pass within that envelope you'll typically need to demonstrate further training and re-apply. Compared with MRCS Part A's longer attempt allowance, FRCS Section 1 is deliberately stricter.

Am I eligible to sit FRCS Section 1?+

Eligibility requires at least 4 years post-graduation in a surgical specialty and demonstrable senior-trainee status — typically ST6+ in UK higher specialty training, or an overseas equivalent (e.g. Indian DM/MCh fellow, post-MS senior registrar with several years in the specialty). Your JCIE application requires documentary proof of training level.

What is the pass standard per diet?+

FRCS Section 1 uses an equated scaled score set by Angoff standard-setting per specialty per diet, NOT a fixed percentage. Crucially, a sectional pass is required on BOTH Paper 1 AND Paper 2 — a strong paper cannot rescue a weak one. Pass rates vary by specialty but typically sit in the 55-75% range per diet.

I've passed Section 1 + Section 2 — what next?+

UK trainees combine the FRCS diploma with a completed run-through training programme to receive their CCT (Certificate of Completion of Training) and entry to the GMC Specialist Register — the licence to apply for substantive consultant posts. Overseas specialists pursue CESR (Certificate of Eligibility for Specialist Registration), where the FRCS diploma is a major evidentiary anchor.
