The short answer is yes — every international medical graduate (IMG), including Indian doctors, needs to pass an English communication skills assessment as part of ECFMG certification. Since USMLE Step 2 Clinical Skills (CS) was permanently discontinued in January 2021, the OET Medicine exam has replaced it as the communication skills requirement. What Happened to Step 2 CS?
USMLE Step 2 CS was a practical exam where candidates demonstrated clinical and communication skills with standardised patients. It was suspended during the pandemic and permanently discontinued in January 2021. To fill the gap, the ECFMG introduced the Pathways system, which includes OET Medicine as the communication skills assessment.
OET Medicine: The New Communication Skills Requirement
All IMGs applying for ECFMG certification must pass OET Medicine, regardless of:
- Native language — even if English is your first language
- Language of instruction at medical school — even if your MBBS was taught entirely in English
- Citizenship — Indian, British, American, or any other nationality

There are no exemptions. OET Medicine is required for every IMG, including those from English-speaking countries.
OET Score Requirements for ECFMG
- Listening: Minimum 350
- Reading: Minimum 350
- Speaking: Minimum 350
- Writing: Minimum 300
All four scores must be achieved in a single test administration. Score combining from multiple sittings is not permitted.
Note that the Writing threshold (300, equivalent to Grade C+) is lower than the other three sub-tests (350, equivalent to Grade B). This mirrors the NMC's writing concession for nurses and reflects the challenge of the Writing sub-test.
How OET Fits into the USMLE Pathway
The typical ECFMG certification pathway for an Indian doctor is:
- Step 1: Pass USMLE Step 1 (now pass/fail).
- Step 2: Pass USMLE Step 2 CK (Clinical Knowledge).
- Step 3: Pass OET Medicine (communication skills requirement).
- Step 4: Meet the requirements of one of the ECFMG Pathways (medical school accreditation or additional clinical experience).
- Step 5: Receive ECFMG certification.
- Step 6: Apply for residency through the Match.
You can take OET at any point during your USMLE preparation — it does not need to be taken in a specific order relative to Step 1 or Step 2 CK. Many doctors prepare for OET alongside Step 2 CK, since the clinical English study overlaps.
OET vs IELTS for ECFMG
ECFMG accepts OET only — not IELTS, TOEFL, or PTE. This is different from GMC (which accepts both IELTS and OET) and AHPRA (which accepts IELTS, OET, and PTE). If you are targeting the US pathway, OET Medicine is your only option.

Preparation Tips
- The OET Medicine exam tests clinical communication, not clinical knowledge. Your USMLE preparation covers the medical content — OET preparation should focus on writing referral letters, conducting patient role-plays, and understanding clinical audio.
- Writing is your most likely weak point despite the lower threshold (300 vs 350). Practise paraphrasing case notes into professional letters — do not copy them verbatim.
- Speaking role-plays are patient consultations, not medical vivas. Use patient-friendly language, show empathy, and structure your consultation clearly.
- Most Indian doctors pass OET Medicine on their first or second attempt with 4 to 6 weeks of preparation.
